NEW PATIENT PROCESS
For your first appointment, please ensure that we have your patient packet 2 business days prior to your appointment.
Please arrive 5-10 minutes early to complete the registration process.
Bring all medication, vitamins, supplements and teas in the original packaging, along with any lab and x-ray reports taken within the last 2 years.
Each new patient appointment includes:
A comprehensive review of your prior medical history and current health concerns.
An overall health assessment addressing physical, emotional, and spiritual wellness with a customized wellness plan. Your plan will be completed based on the complexity of your health issues. Individualizing your care may require research time.
A hands-on physical exam with laboratory testing as indicated.
Supplements and herbs are available for purchase. Credit cards are accepted.
Follow-up consultations to review tests and revise your wellness plan are usually 60 minutes. Payment due at the time of service.
Cancellation Policy: Our goal is to provide quality care for all of our patients in a timely manner. We allot a significant amount of time for each patient and appointments are in high demand. We keep a waiting list of patients requesting to move up their appointments to earlier dates. Therefore, cancellation notices for new appointments must be received by our office at least 2 weeks in advance.
Initial appointments require payment in full at the time of scheduling. Andrs Wellness requires a credit card on file at all times to cover any cancellation fee. A patient's card will ONLY be used for this reason unless the patient gives us permission to charge their card for subsequent appointments or for merchandise. If a patient cancels their appointment without providing at least 2 weeks notice, their card will be charged the full price of their appointment.
Payments are 100% non-refundable unless one of the following takes place:
The patient cancels 2 or more weeks in advance of their appointment date, in which case, they will receive a full refund, less a $50 office processing fee.
The patient reschedules their appointment at least 2 weeks in advance of their original appointment. In that case, there will be no fee.
